The Great Rewiring

Every generation of investors faces the same challenge: recognising structural change before it becomes conventional wisdom. Most innovation improves existing industries. Occasionally, however, an entire industry is reorganised around a new operating model, creating entirely new sources of long-term value for those who recognise the shift early.
Healthcare has reached one of those moments.
For more than a century, healthcare has been built on a remarkably successful model. Scientific discovery drove medical progress, clinicians became the custodians of specialist knowledge, and healthcare systems evolved to deliver those advances at ever greater scale. The result was one of the greatest improvements in human welfare ever recorded. According to the World Health Organization, global life expectancy increased by more than thirty years during the twentieth century, driven by advances in public health, vaccination, antibiotics and modern medicine. Yet the assumptions that underpinned that model are beginning to change.

Healthcare systems are under growing pressure. According to The King’s Fund and the Institute for Fiscal Studies, ageing populations, rising rates of chronic disease, workforce shortages and constrained public finances are placing increasing demands on healthcare systems while limiting their ability to expand capacity. Demand continues to outpace supply, exposing the limits of a model built for a different era.

At precisely the same time, science continues to advance. Artificial intelligence is already accelerating drug discovery, while precision medicine is moving from research into clinical practice. Genomics, digital diagnostics and connected care are expanding what medicine can achieve, creating new opportunities that healthcare systems are increasingly struggling to absorb.

The paradox is becoming increasingly difficult to ignore. Advances in artificial intelligence, precision medicine and genomics are expanding what medicine can achieve, yet healthcare systems continue to struggle to translate those advances into routine patient care.

That simple observation captures one of the defining challenges facing modern healthcare. The greatest constraint is no longer scientific discovery alone, but the ability to translate innovation into routine clinical practice. When the principal constraint within an industry changes, so too does where value is created.

Healthcare is not simply experiencing another wave of innovation. It is being fundamentally rewired.
The Science Is Ahead of the System
For most of modern medicine, progress depended on scientific discovery. Better science led directly to better healthcare, and investment naturally gravitated towards the organisations developing the next breakthrough therapy, diagnostic or medical device. That relationship is beginning to change. Artificial intelligence is already accelerating drug discovery, while advances in genomics, precision medicine and diagnostic technologies are moving steadily from research into clinical practice. GLP-1 therapies such as Wegovy and Mounjaro have transformed the treatment of obesity, while disease-modifying Alzheimer’s therapies including donanemab and lecanemab are beginning to redefine what is possible for neurodegenerative disease. Yet years often separate scientific validation from routine patient care.

The bottleneck has shifted. Increasingly, the challenge is no longer invention, but implementation. Scientific discovery remains the foundation of healthcare progress. Increasingly, however, competitive advantage is shifting towards organisations capable of translating innovation into routine clinical practice. The challenge is no longer simply proving that new technologies work, but integrating them into healthcare systems, generating real-world evidence and enabling adoption at scale. Execution is becoming as valuable as invention.
The End of the Top-Down Era
This transformation is not being driven by technology alone. It is reshaping the relationship between patients, clinicians and healthcare systems. For decades, knowledge flowed in one direction. Healthcare institutions generated expertise, clinicians acted as its custodians, and patients were largely passive recipients of care. That model is beginning to give way to something fundamentally different.

Patients are becoming active participants in managing their own health. The widespread adoption of technologies such as Oura, Whoop, Zoe and continuous glucose monitoring, together with the rapid emergence of AI-enabled health assistants, reflects a broader shift towards individuals taking greater ownership of their health. Care is increasingly extending beyond hospitals into homes, workplaces and communities.

This does not diminish the role of clinicians. On the contrary, technology is likely to make clinical expertise more valuable by automating routine tasks and allowing healthcare professionals to focus on diagnosis, judgement and patient care. Rather than replacing clinicians, it is extending their reach. The top-down model is ending.
Building Healthcare’s New Infrastructure
During his keynote at the Mayo Clinic Platform Conference, Matt Hancock described the Mayo Clinic Platform as: “The data platform I wish we had created in the NHS.”

The observation captures an important shift in modern healthcare. Data is no longer simply a by- product of healthcare delivery; it is increasingly becoming the infrastructure upon which healthcare systems operate. Organisations such as the Mayo Clinic are demonstrating how connected clinical data can accelerate research, improve diagnosis and support the adoption of new technologies across routine care.

High-quality clinical data underpins precision medicine, predictive analytics and the next generation of healthcare platforms. Connected patient records enable more personalised care, while the ability to analyse millions of patient interactions is creating new opportunities for earlier diagnosis, disease prevention and more effective treatment. 
The organisations defining the next generation of healthcare will not necessarily be those making the next scientific breakthrough. Increasingly, they will be those building the platforms, systems and capabilities that enable scientific discovery to improve patient care at scale.
Building Healthcare’s New Infrastructure
Every major shift in healthcare changes where value is created. The current transition will be no different. The defining healthcare businesses of the next decade are unlikely to be distinguished solely by breakthrough science. Increasingly, they will be distinguished by their ability to translate scientific innovation into scalable clinical value, bridging the gap between discovery and delivery.

For Havilland, this investment thesis shapes how we think about healthcare. Competitive advantage will increasingly belong to businesses capable of combining exceptional science with the execution required to deliver meaningful clinical and commercial impact. The history of healthcare has largely been written by scientific discovery. The next chapter will increasingly be written by those who enable its adoption.
